Mental Health Services / Facilities: Psychiatric Case Management
SERVICE DESCRIPTION
Offers case management services for severely mentally ill clients. Services include assistance in linkage, resource identification, supportive counseling, daily living skills, building a supportive social network, and client advocacy.

Accepts Medicaid/Soonercare, Medicare, insurance; sliding scale.
ELIGIBILITY
Must have severe mental illness and/or co-occurring.
DOCUMENTS REQUIRED
Must provide photo ID/driver's license, last two payroll statements (for self and spouse if married), any recent hospital discharge papers for mental health admissions, written list of all medications (name, strength, amount taken, frequency, what it treats and prescribing doctor).

SERVICE DESCRIPTION
Provides home-based mental health services to children and youth who have severe emotional or behavioral disturbances that place them at risk for out-of-home or out-of-school placement. Builds supports and resources within the community to keep youth at home/school safely.

Must be 25 years of age or younger, have behavioral/mental health issues involved with two or more systems (school, JSU, Mental Health, DHS, Juvenile Drug Court, Probation and Parole), and be at-risk for out-of-home placement or out-of-school classroom placement.
